How to Find a Fax Number for a Doctor, Insurer, or Agency
Fax numbers are not as easy to find as phone numbers, and a wrong one can send your records to a stranger. Use a trusted source and confirm it.

Where to Look
- Call the office. The most reliable. Ask for the fax number for the department that needs your document.
- The organization's own website. Look at the contact or forms page, not a third-party listing.
- The form or letter you were given. Many forms print the number to return it to.
- Your insurance card or member portal. Claims and prior-authorization numbers are often listed there.
- A patient portal. Many have a way to send documents securely, which can replace a fax.
What to Be Careful About
Third-party directories can be out of date or wrong. An old or incorrect number may reach a different business. For anything sensitive, do not rely on a number you cannot trace back to the organization itself.
Confirm Before You Send
Then Send It
